– Submit one note and treatment plan
– Goals must be written in SMART format (pay close attention to measurability and time – these are two different things)
Negotiables:
The type of note you submit is up to you. You can choose to complete any type of note discussed in Week 14 (SOAP, PIE, DAP).
You do not have to keep the same presenting problem that you’ve been using in roles plays over the past week. Feel free to change it up.
If you utilize a template for notes and treatment plans at your current agency, feel free to use it. You do not have to utilize the templates that are provided to you.
*Reminder that notes should be concise and do not need every specific detail of your session
I will attach the template as well as the template example. The textbook is attached as well, along with any other relevant info.
